The customer paid for more credits than the product granted.

Identity mismatch
Checkout identity does not match the application subject that received access.
Renewal anomaly
Renewal succeeded while access expired, or cancellation left premium access in place.
Contract overlap
An enterprise seat and a personal subscription can bill overlapping access.
Revenue exposure
Currency-separated integer amounts attached to findings. Never mixed. Never a promised recovery.
